Needs and Project Objectives
According to the 1990 census, Chemung County has 95,195 residents.
The county has a higher percentage of children (up to age 14)
and senior citizens (age 65+) than upstate New York as a whole.
Median household income was $26,135 in Chemung County as compared
with $35,712 for upstate New York. A higher percentage of Chemung
County children (24 percent) qualify for and are receiving
Medicaid benefits than children residing in upstate New York
overall (18.2 percent). More than 30 percent of the population
of Chemung County was below 20 percent of poverty. And in Chemung
County, the rate of teenage pregnancy is 115.1 per thousand
population as compared with 73.2 per thousand for upstate New
York.
Using a broad-based community health needs assessment, The
Chemung Valley Health Network has developed a series of project
objectives which form the basis of a detailed workplan to
improve community health within Chemung County.
Needs Assessment
Needs as identified by the members of the Health Network
are defined by the prevailing conditions within our county:
- The high rate of teenage pregnancy
- The high incidence of low birthweight
- Access to primary care
- Recruitment and retention of primary care physicians
- Access to dental care, particularly for Medicaid and indigent
populations
- Access to mental healthcare, particularly for children
- Availability of HIV care
- Lower than desirable immunization rate
- Lack of community knowledge and awareness of services available
- Lack of coordination of services

Chemung County Health Department Registered Nurses
Chemung County Health Department is currently seeking enthusiastic
professional nurses to provide Home Care to Chemung County Residents.
We are committed to meeting the home health care needs of our
community by providing the best patient care and making it possible
for patients to remain where they prefer to be - in their homes.
The Chemung County Certified Home Health Agency offers an
autonomous work environment and is ideal for those who like
to work independently. Home Health work provides nurses with
the opportunity to build relationships, and make a difference,
one patient at a time.
Schedules are family flexible and typically are set for 8
hours per day, five days per week with weekend requirements
once every three weeks. No more 12 hour shifts!
We offer one of the best benefit plans in the region: New
York State Retirement System, comprehensive medical/dental/vision
and drug plan, generous benefit time package, mileage, uniform
and cell phone reimbursement and tuition assistance. Starting
hourly rate is $23.02-$25.39 depending upon experience.
Qualified candidates will have registered nurse licensure and
a minimum of two years full time, direct, patient care experience.
Acute care nursing experience and the ability to pass a computer
competency assessment are also required.
The ideal candidate will be able to work independently with
limited direct supervision and possess strong clinical assessment,
organizational and communication skills.

Help the
Uninsured Get a Quick Start on Rx Savings
With the quick start savings card from Together Rx Access, individuals
who lack prescription drug coverage and are not eligible for
Medicare don't have to wait to start saving on the medicines
they need. Individuals and families can determine if they qualify
for the quick start savings card by calling 800-250-2839. If
eligible, the card can be used at the pharmacy counter that day.
Most cardholders save 25% to 40% on brand-name prescription drugs
and products with the free-to-get and free-to-use Card. The program
includes more than 300 brand-name medicines and products, prescribed
to treat diabetes, hypertension, high cholesterol, allergy, asthma,
arthritis, depression, and other common conditions. Savings on
a range of generic medicines are also available. Email Amy Niles

cards that you can distribute to those who may be eligible.
