The Division of Health and Long Term Care determines Medicaid eligibility for people in the community and in nursing homes, oversees Medicare Maximization efforts; Managed Care and Comprehensive Medicaid Case Management programs. In addition, this division employs registered nurses and caseworkers who conduct comprehensive long term care assessments in homes and hospitals, and develop plans of care which enable individuals to receive Long Term Care Services in appropriate settings in the community.
