Southern Tier Hospice is a not-for-profit, community-based, Medicare
certified program providing complete hospice services to residents
of Chemung, Schuyler and
Steuben Counties in New York State.
Two interdisciplinary teams, one based in Elmira, NY and one
in Hornell, NY, provide individualized care to patients and their
families, primarily in the home. Each team consists of physician,
nurses, social worker, pastoral care coordinator, volunteer coordinator,
bereavement coordinator, and trained volunteers, in cooperation
with the patient's personal physician, provides care for the patient
and support for the family.
