Assessment of Community Needs and Project Objectives
Lack of community knowledge and awareness of services available
While Chemung County has more innovative programs and services that address health-related needs than most rural areas, it seems that the more services that become available the more difficult it is to build awareness among members of the community most in need. The Southern Tier Primary Care Project identified this as the most critical barrier in delivery of care. In forming, members of the Health Network recognized the complexity and diversity of the maze of services currently available within the Chemung County area, and the challenge faced by any individual seeking to navigate the many delivery system pathways.
