With support from the New York State Department of Health, the Chemung Valley Health Network has formed with the purpose of:

1. Improving the health status of all Chemung County residents receiving health care services by:
   • adhering to measurable outcomes
   • encouraging innovation
   • ensuring responsiveness to community needs
   • utilizing needs assessments to assure appropriateness
   • promoting prevention as a priority
   • cultivating collaboration

2. Overcoming barriers that impede access to healthcare services by:
   • providing services that reach into rural areas
   • encouraging and supporting advocacy for improvement of the healthcare delivery system
   • workng towwared guaranteed access to healthcare for all regardless of location or income
   • ensuring comprehensive care
   • utilizing community, patient and family input in designing the healthcare delivery network of the future

3. Promoting knowledge about health and wellness in our community by:
   • Offering easily accessible health and wellness education through multiple channels to agencies and individuals alike
   • Promoting and enabling people to practice informed self-help and self-care

4. Ensuring quality of healthcare services by:
   • Emphasizing the continuity of healthcare services
   • Actively recruiting and retaining quality healthcare providers
   • Promoting provision of care in the most appropriate settings

5. Ensuring financial integrity of the system by:
   • Advocating proactive regulation planning
   • Supporting shared risk
   • Eliminating duplications of service in the system
   • Upholding cost controls
   • Giving precedence to efficiency

6. Developing and promoting a health-related network that:
   • ensures inclusion of all providers
   • promotes trust and cooperation among all providers
   • fosters blending of services
   • develops awareness of other service/initiatives
   • cultivates linkages
   • develops electronic information sharing
   • allows for dynamic development of the health network
